1. Print and complete the AUTHORIZATION FOR MEDICAL TREATMENT.
2. Mail or fax the completed Authorization for Medical Treatment form to the following address or fax number:

3. Visit the ALL-STATE CHOIR CAMP REGISTRATION page.
4. Select one of the following options and click ADD TO CART (2009 All-State Choir Members receive FREE tuition):
   

   A. Regular Camper: $210 ($130 room/board, $80 Tuition)
   B. Commuter: $80 (tuition only)

5. Complete the secure online registration form, payment information, billing address, and click SUBMIT.
6. You will receive an automatic email confirmation. A second email confirmation will be sent once your credit card payment has been processed.
